CV-22 Accident Investigation Report Released

Inadequate Risk Management And Ineffective Crew Resource Management, In Part, To Blame

Air Force Special Operations Command officials released the results of the investigation of the CV-22B Osprey mishap Nov. 29, 2023, near Yakushima, Japan. The aircraft, assigned to the 21st Special Operations Squadron, 353rd Special Operations Wing at Yokota Air Base, Japan, was participating in a joint inter-operability exercise when the mishap occurred. The aircrew was comprised of personnel from the 21st SOS, 1st SOS and 43rd Intelligence Squadron.

Historic FAA Authorizations Permit Drone Deliveries Without Visual Observers

More Dallas-area Authorizations Anticipated

The FAA made a little aviation history in the U.S. when it announced the issuance of authorizations for multiple commercial drone delivery services to operate simultaneously in the same airspace without the typical requirement for visual observers beyond visual line of sight (BVLOS). Drone flights currently require the pilot or trained visual observers (VOs) to maintain visual contact with the drone during flight. However, these advances in technology and procedures are key to enabling the routine performance of BVLOS drone flights.

NTSB Prelim: Fantasy Air SRO Allegro 2007

The Airplane Came To Rest In A Nose Down Attitude, In An Open Prairie On A Heading Of About 120° Magnetic

On July 7, 2024, about 1800 mountain daylight time, a Fantasy Air SRO Allegro 2007, N969RC, was substantially damaged when it was involved in an accident near Eureka, Utah. The pilot was fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. Review of preliminary ADS-B derived data provided by commercially available source, showed that the airplane departed Spanish Fork Municipal Airport/Woodhouse Field, Spanish Fork, Utah at 1558. The data showed that the airplane flew on a westerly heading and maneuvered within the vicinity of Eureka.
