Source: Col. Daniel O'Toole, USAF (Ret.). talking about his son, Col. Michael J. O'Toole, USAF, and his fatal crash in a Cessna 182. The crash took his life but spared those of his wife and daughter. The NTSB has just released the final report on the crash.
