Liquid Oxygen (LOX) Tank On Booster 18 Ruptured While Testing

By Wes Oleszewski- ANN Spaceflight Analyst SpaceX has suffered what could be a large set-back in its Starship Program. On November 21, the Liquid Oxygen (LOX) tank on Booster 18 ruptured while testing. This is the first of the Block 3 Starship booster stages, which are the third generation of the huge Starships that have recently launched. The failure took place at SpaceX’s Masseys test facility in Texas. Since at least 2006, the USFS had a single source supplier for LTFRs that provided retardants with monoammonium phosphate (MAP), diammonium phosphate (DAP), and/or ammonium polyphosphate (APP) used as the retardant salt. Beginning in 2020, the USFS allowed LTFRs to be submitted for qualification that contained retardant salts different from the historic phosphate-based retardants.

Airplane Lunged Forward When It Was Stuck From Behind By A Tug That Was Towing An Unoccupied Airliner

Analysis: At the conclusion of the air taxi flight, the flight crew were taxiing the small twin-engine airplane to their assigned gate at night. The crew were holding short of the gate area and awaiting clearance to enter. After holding for about 10 minutes with the airplane’s parking brake set, the flight crew heard a loud noise and the airplane lunged forward when it was stuck from behind by a tug that was towing an unoccupied airliner. The small airplane sustained substantial damage to its elevator and horizontal stabilizer. The tug’s windshield was damaged during the collision, and the airliner was not damaged.

Smoke Began Entering The Cockpit During The Landing Flare, And Then The Pilot Noticed Flames On The Right Side Of The Airplane

Analysis: The pilot reported that about 30 minutes into the local flight, he heard some crackling in the radio reception and then the cockpit flight displays went blank. The engine stumbled and lost power completely. The pilot was operating the airplane in the airport traffic pattern at that time and decided to secure the engine to set up for a forced landing to the runway. The pilot and sole occupant of a Cessna 172 was killed when the aircraft was involved in a mid-air collision above the town of Martintown in southeastern Ontario. The aircraft went down in a wooded area and the pilot, a 28-year-old man from Île-Bizard, Quebec, was found dead. The other aircraft, a Piper Seminole with two on board, was able to land safely at Cornwall Regional Airport (YCC/CYCC).

According To The Witness, Once The Airplane Landed, It Continued To Roll In A Relatively Straight Line Until It Impacted A Tree In His Front Yard

On November 4, 2025, about 12:45 eastern standard time, a Funk B85C airplane, N24157 was destroyed when it was involved in an accident near Anderson, Indiana. The pilot was fatally injured. The airplane was operated under the provisions of Title 14 Code of Federal Regulations Part 91 as a personal flight. The airplane departed a private grass airfield (II26) in Wolcott, Indiana, earlier on the day of the accident and was enroute to Anderson Regional Airport (AID), Anderson, Indiana. The airplane was recently purchased, and the pilot was flying the airplane to AID for the new owner.
