Cirrus Aircraft has seen yet another SF50 Vision jet suffer a runway accident in which the vehicle encountered a failed takeoff at the Colombia, Santa Marta Airport. The SF50 Vision Jet had four persons aboard... all of whom reported some degree of injury but a minor child was reportedly struck by the jet, and killed, as it attempted to depart Runway 01. NTSB Closes In On Suspicious Issues In DHC-3 Crash

Identifies Possible Safety Issue in Tragic Mutiny Bay Accident

National Transportation Safety Board investigators identified a possible safety issue with a flight control of the De Havilland Canada DHC-3 that crashed into Mutiny Bay, Washington on Sept. 4, 2022. Tragically, the pilot and nine passengers were fatally injured. Investigators found (recovered airframe parts shown in green) the horizontal stabilizer actuator, which is part of the airplane’s pitch trim control system and is also referred to as the trim jack, had separated into two pieces.

"CNATRA Made The Decision To Halt All T-45C Goshawk Operations..."

The Chief of Naval Air Training (CNATRA) placed the Navy and Marine Corps’ fleet of T-45Cs on a safety pause several days ago in order to review an engine blade fault. "Out of an abundance of caution and concern for the safety of our aviators, CNATRA made the decision to halt all T-45C Goshawk operations following the discovery of an engine blade failure,” said CNATRA Rear Adm. Richard Brophy.

AD 2022-17-04 Prompted By Reports Of Broken P-Clamps On The Pressure Relief Line

The FAA is adopting a new airworthiness directive (AD) for certain De Havilland Aircraft of Canada Limited Model DHC-8-400 series airplanes. This AD was prompted by reports of broken P-clamps on the pressure relief line and the motive flow line in the fuel tanks, and a subsequent determination that certain service information lacked instructions for maintaining appropriate clearance between certain fuel tubes and their support brackets, and may also have led to incorrect installation of certain TeflonTM sleeves. This AD was also prompted by a determination that new or more restrictive airworthiness limitations are necessary.

NTSB Prelim: Scoda Aeronautica LTDA Super Petrel LS

Accident Airplane Abruptly Pitched Up, Descended To The Right And Impacted The Water

On October 2, 2022, about 0900 Central daylight time, a Scoda Aeronautica LTDA, Super Petrel LS, N892SP, was substantially damaged when it was involved in an accident near Perma, Montana. The pilot was sustained fatal injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. According to a witness, he and the accident pilot departed Polson Airport, Polson, MT, about 0800, as a flight of two.
