The NTSB has determined pilot error led to an American Airlines
flight attendant being sucked out of an airplane following an
emergency landing in Miami more than five years ago.

SUMMARY: The FAA is adopting a new airworthiness directive
(AD) for all Boeing Model 747-200F, 747-200C, 747-400, 747-400D,
and 747-400F series airplanes.
They would prefer not to be there in the first place, and
they're probably gonna lose money on every plane that comes
through. It was with this cheery outlook that American Airlines
began flying from Dallas' Love Field Thursday, competing with
Southwest Airlines on its home turf for the first time since
2001.
Virgin Atlantic has launched an investigation into claims a
flight attendant aboard a flight from London-Gatwick to Las Vegas
earlier this week panicked when the Boeing 747 hit severe
turbulence.
