The National Transportation Safety Board detailed in an accident report issued Tuesday the circumstances that led to the crash of a Boeing B-17G airplane that killed seven people and injured seven others. The NTSB determined the probable cause of the accident was the pilot's failure to properly manage the airplane's configuration and airspeed following a loss of engine power. Safety issues found during this investigation were discussed during a March 23, 2021, public board meeting on Part 91 revenue passenger-carrying operations.

NS-15 Launched April 14 With Great Success

Blue Origin successfully completed its 15th consecutive mission to space and back Wednesday and conducted a series of simulations to rehearse astronaut movements and operations for future flights with customers on board. This mission marked a verification step prior to flying astronauts. For the first time, Blue Origin personnel standing in as astronauts entered the capsule prior to launch. These astronauts conducted a series of tests from within the capsule, including a comms check with the Capsule Communicator (CAPCOM), procedures for entering and exiting the capsule, and pre-launch preparations within the capsule.

Expedition 64 Flight Engineer Kate Rubins of NASA and two Roscosmos cosmonauts are scheduled to end their mission on the International Space Station Friday, April 16. Rubins, along with Russian cosmonauts Sergey Ryzhikov and Sergey Kud-Sverchkov, will close the hatch to the Soyuz MS-17 spacecraft at 6:10 p.m. EDT to begin the journey back to Earth. After Entry Into The Canyon, The Airplane Turned Sharply To The Right And The Witness Lost Sight

On April 3, 2021, about 1735 central daylight time, a Magnus Fusion 212 airplane, N434MA, was substantially damaged when it was involved in an accident near Conifer, Colorado. The sport pilot was fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. A review of air traffic control (ATC) information revealed the airplane departed about 1658 from Colorado Air and Space Port Airport (FCO), Denver, Colorado. The airplane flew southwest and the last ATC information was recorded about 1729 as the airplane flew near Deckers, Colorado about 800 ft above ground level (agl).
